Reason:
Alexander has apparent needs in the areas of speech/language and sensory that needs to be evaluated/addressed.
Evaluation Summary:
Fine motor: Alexander demonstrated many characteristics of a child with low registration. This means that he is missing sensory stimuli that provide cues to what is going on around him. He may appear uninterested and have low energy levels. Alexander will benefit from skilled occupational therapy to help set up a more sensory stimulating environment and perhaps begin a ‘how does your engine run” program.
Speech/Language: Alexander’s social language and attention skills are below average. He also demonstrates occasional choppy speech (when he can’t process and formulate thoughts quickly) and flat intonation. He demonstrates needs in the areas of understanding the physical context, understanding and considering other people’s feelings/expressions and purpose, and politely interacting communicating with others.
Services
Special education 1/18/2010 – 11/19/2010. 235 minutes per week.
Speech/Language Therapy 3/30/2010 – 11/19/2010. 270 minutes per quarter.
Notes:
Consultation Speech Language Service: 15 mins per week, Occupational Therapy Consult: to develop a sensory program, then quartly to monitor. Sensory Strategies: blend into daily.
